There has been a change in contact person for cases involving discontinuances or denials of participation in the Home and Community Based Services Waiver program for individuals with traumatic brain injuries (TBI). These are cases coded against NMAP (if NYC) or the LDSS, with a secondary agency of DOH1. The new new contact information for the NYS Department of Health (DOH) is as follows:
