Effective October 1, 1999, Fair Hearing requests involving Income Support Center 52, Office of Treatment Monitoring, will be held at 109 East 16th Street.
Attached to this transmittal is an information sheet which provides New York City Hearing Locations By Agency, which has been updated to reflect this change. These agencies will appear in the first Agency field on FHIS, which dictates the hearing location.
