This transmittal announced that Report FH-019, referred to as the "Federal Data Sheet," has been revised to include the Client Notice System (CNS) Notice ID Number.
A new FH-019 will be created each time a Fair Hearing is requested or rescheduled as well as when a Supplemental or Amended Issue is entered on the Fair Hearing Information System (FHIS).
