This transmittal explains that a new issue code has been created in FHIS (Fair Hearing Information System): 265 Pharmacy Issues.
For pharmaceutical determinations involving prior approval/authorization by the New York State Department of Health, Office of Health Insurance Programs (OHIP), a new Agency field (BPPO) has also been created in FHIS, which will notify:
